FERPA Privacy Waiver: This form is used to allow Carnegie Mellon to share your grades with a designated person.

Transfer Credit Request Form: You must complete this form if you are requesting to transfer credits from another institution.

CIT Program Request Form: This form is used by current Carnegie Mellon students who are interested in transferring to a program within the College of Engineering.

CIT Minor Request Form: This form is for students who wish to pursue minors offered by the College of Engineering.
